Registration Mechanics:
1. This registration form is open to all pregnant mothers who wish to participate in either of St. Luke's Medical Center's events and can personally claim their prize from Cordlife Philippines at our booth.
2. To qualify, the pregnant mother registrant must not have her details existing in the Cordlife Philippines database.
3. The first 5 qualified mothers will be notified through SMS or email and should personally claim their prize from Cordlife Philippines at the event, either at SLMC Quezon City or SLMC Global City.
